Basic Legal Requirements
- Must be a resident of Rhode Island and must have proof of residency or valid ID
- Must be 18 years or older. Minors need parent or guardian permission
Proof of Qualifying Conditions
- Patient’s condition must be qualified
- Must posses medical records of condition
- Must have doctor fill out a Practitioner Form
Complete the Paperwork
- Must fill out application form for Medical Marijuana Card
- Must pay a nonrefundable $75 fee to apply for a medical marijuana card. If on Medicare, SSI, or SSDI, the cost will be reduced to $10
